For potential clients (leads), you may just want to get the minimum amount of information you can depending on how you intend to contact them with it. A client expects to fill out all the above information. But the lead will be uncomfortable giving out lots of personal information without a good reason. So get some minimum contact information, or give them a perfect reason for needing more information (entering contests, getting newsletter, etc). You can do all of this without software, but it becomes much more difficult. Particularly to do physical mailings. So I really suggest you get with it and computerize your mailing and list process.
